At Your Community Healthcare Center, LLC (YCHC), we care for our community with compassion, dignity, and understanding. As nurses, we believe healing begins with being seen, heard, and respected. Our mission is to support the health and growth of young adults and adults ages 18 and older by providing culturally grounded, community-focused care that meets people where they are.

We are especially committed to walking alongside individuals with a history of substance use, offering nonjudgmental support, education, and hope as they move toward stability and long-term wellness. Our approach centers on building trust, strengthening resilience, and promoting whole-person healing—mind, body, and spirit.

Our team is made up of registered nurses from diverse walks of life, faiths, cultures, races, and backgrounds, including members of the Nation of Islam. Together with nurse practitioners, psychiatrists, social workers, peer specialists, and dedicated volunteers, we serve with love, honor, and respect—because our community deserves care that is both skilled and deeply human.

We walk alongside each individual with personalized support, offering a full spectrum of services, including:

  • Case management and life coaching

  • Educational workshops, job training, and apprenticeships

  • Transitional and permanent housing support

  • Medication-Assisted Treatment (MAT)

  • Mental and behavioral health services and medications

  • Nutritional health education and healing cooking classes

  • 1:1 Healthy Gut Training Coach 

  • Physical fitness and wellness programs

  • Financial literacy and empowerment

  • Spiritual renewal and holistic care

We believe in whole-person recovery and the power of peer-driven support. Whether you're just beginning your journey or continuing your path to wellness, you are not alone—we are here with you, every step of the way.

We also extend our mission through professional partnerships, providing contractual services to home health care agencies, long-term care facilities, substance use programs, and behavioral health centers.

Program Overview
ROOTS & RESTORATION is a weekly peer support group designed to provide a safe, culturally rooted, and healing-centered space for family members and loved ones of individuals who are living with or recovering from substance use disorder (SUD).
This group recognizes that substance use impacts entire families and communities, not just the individual. Through peer connection, education, and holistic support, participants are empowered to heal, grow, and strengthen family bonds while building resilience and self-sufficiency.

Program Goals
The ROOTS & RESTORATION Peer Support Group aims to:
•    Provide emotional support and validation for loved ones affected by SUD
•    Promote family reunification, restoration, and healthy boundaries
•    Reduce stigma, isolation, and caregiver burnout
•    Strengthen coping skills, communication, and self-care practices
•    Offer education on substance use, recovery, mental health, and wellness
•    Connect families to YCHC services and community resources
•    Foster hope, healing, and empowerment through peer-driven support

Who Can Attend
This group is open to:
•    Parents and caregivers
•    Children and adolescents (age-appropriate breakout sessions available)
•    Siblings
•    Spouses and marital partners
•    Extended family members
•    Close friends and support persons
Participants may attend whether their loved one is currently using substances, in treatment, in recovery, or no longer living.

•    Frequency: Weekly
•    Length: 90 minutes
•    Setting: In-person and/or hybrid (virtual option available)
•    Facilitators: TBA
o    Trained Peer Support Specialist
o    Licensed Nurse / Social Worker (as appropriate)
•    Approach: Trauma-informed, culturally responsive, spiritually grounded, and strength-based
